Ambulatory Care Clinics

The Patient Aligned Care Teams (PACT) are comprised of providers, registered nurses, licensed practical/vocational nurses, health technicians, and medical support assistants who provide primary care and coordinate specialty care and other services. Primary Care is the provision of integrated, accessible healthcare services by clinicians who are accountable for addressing a large majority of personal healthcare needs, developing a sustained partnership with patients, and practicing in the context of family and community. It includes intake assessment, health promotion, disease prevention, Women’s Health, management of acute and chronic bio psychosocial conditions, referrals for specialty, rehabilitation, and other levels of care, follow-up and overall care management, and patient and caregiver education. This service is provided at the hospital campus in Roseburg to all eligible Veterans residing in the Roseburg geographic area. The Roseburg campus resides in Douglas County and serves Veterans in this and adjacent areas. The Eugene CBOC serves primarily Veterans living in Lane County. Some Veterans also come from Linn and Benton counties which neighbor Lane County to the north. The North Bend and Brookings CBOCs serve Veterans in coastal counties of Oregon and northern California.

The Associate Chief of Staff for Ambulatory Care provides clinical oversight for the scope of practice of licensed independent and dependent providers and unlicensed assistive personnel (Medical Assistants and Health Technicians).

The Associate Chief Nurse Executive/Ambulatory Care Nursing provides clinical oversight for the scope of practice for RN, LPN, and unlicensed assistive personnel (Health Technicians) in collaboration with the ACOS/Ambulatory Care. The PACT staffing model includes one RN Care Manager per provider and one back office support (either LPN or Health Tech) per provider. An RN Nurse Manager provides administrative and clinical oversight of nursing personnel at each site and reports to the Associate Chief Nurse Executive/NP Ambulatory Care Nursing, who reports to the Associate Director for Patient Care Services/Nurse Executive.

Ambulatory Care clerical support in Ambulatory Care Operations Manager that oversees clinical operations at all sites who reports to the Associate Director. Under the Ambulatory Care Operations Manager there is an operations manager at each of the clinics, with the exception of North Bend and Brookings who share one operations manager that is located in North Bend. The clerks at each of the clinics reports directly to the Operations Manager for that particular clinic.

Hours of Operation:
Primary Care is available Monday through Friday 8:00 a.m. – 4:30 p.m. For sites with 10,000 Veterans, extended hours are on Tuesdays from 4:30-6:30pm (Eugene and Roseburg), Wed, Thurs, and Fri from 7:00-8:00am (Roseburg only), Saturday 8-4:30pm (Eugene only) and Saturday from 8-12pm (Roseburg only)
